When your coverage will start?

Available plan states: Arizona, California, Colorado, Delaware, Florida, Georgia, Idaho, Illinois, Indiana, Iowa, Kansas, Maryland, Michigan, Minnesota, Nebraska, Ohio, Oklahoma, Oregon, Pennsylvania, South Carolina, South Dakota, Tennessee, Texas, Virginia, and West Virginia.

I will be 60 years old (or older) as of my coverage effective date. Coverage will be effective on the 1st of the month following the day you complete your enrollment.

I understand that enrollment in this plan requires a 12-month commitment to the plan.

I also understand that anytime I cancel this coverage there will be a minimum 12 month waiting period before I can enroll in these coverages again.
